In Serling’s episode, â€Å"The Masks† in the famed tv show, The Twilight Zone, Jason Foster is the dying old rich man whose views of his family members are most beneficial and realistic for the viewer as he forces them to wear grotesque masks which inevitably is the antithesis of what the family sees themselves as. The realistic personalities of the family members are brought to the surface rather than the family members’ worldviews of selflessness, greed, and immaturity, as Foster permanently engrains the masks of reality into their true faces. The first way in which the masks are used to showcase as to why Jason Foster’s views of his family members are more realistic starts from the first incident where he reconciles with his family only at the ending of his life. Alas, the children, Paula Harper, the self-centered daughter who only cares about one thing only, the materialistic things of beauty and lastly, Wilfred Jar for lack of a better phrase is nothing but a sack of potatoes who was called an animal torturer. The motives of the family are clearly painted in the specific scene in the show, where Jason foster brings up his clever idea of wearing the masks during Maris Gras. Immediately, Paula and Wilfred Jr shut down the idea by putting both of their say in the situation. Yet, the parents (which are not really good parents) don’t even acknowledge and respect the idea of their old man but just let it linger. In fact, Wilfred states â€Å"we are at odds her, father† thus Jason Foster snaps into action and pin points the real reasons as to why the family decided to show up for a visit the day he is dying- to collect his fortune. Not denying it, Jason convinces the family Togo through with his plane by bribing them with his fortune. That right there shows the lack of compassion and the real movies of the family, especially the husband, Wilfred. In the end, the dying man, Foster takes care of his last set of business, by giving justice to the views. As this is all taken place in one room during Mardi Gras, at the end he yells viciously, â€Å"Without your masks, you are all caricatures! † This last set of words Foster exclaims. His haunting words showcases the reason for his voodoo tricks, to make the family realize who they really are. Jason Foster truly does unravel the views of his family members perfectly, as it goes to show most people even family have ulterior motives filled with greed and care about the betterment of themselves only!

Spirituality and Sexuality in Palliative Care Anjum zahoor Significance of spirituality and sexuality in palliative care According to WHO (2012) â€Å"Palliative care is an approach that improves the quality of life of patients and their families facing the problem associated with life-threatening illness, through the prevention and relief of suffering by means of early identification and impeccable assessment and treatment of pain and other problems, physical, psychosocial and spiritual†. The aim of palliative care is to provide comfort care and support system at end of life, to relieve suffering and cope with problem associated with life changes due to illness. Literature also supported that Palliative care does not depend on prognosis, as the life is at end stage, or cure the disease. But it focuses primarily on anticipating, preventing, diagnosing, and treating symptoms experienced by patients with a serious or life-threatening illness and helping patients and their families make medically important decisions. During my palliative course clinical I encountered with 80 year old male patient admitted with complain of drowsiness, hiccups and upper GI bleed. At Aku he was treated as aspiration pneumonia. During history taking patient stated that, from two days I was feeling weak and unable to move myself. After investigations, reports revealed that he was suffering from bilateral brain atrophy, subdural hygromas and Pleural effusion in left lung. In Four days clinical I observed that his son took care of his father, reciting holy Quran and sprinkle holy water on his face. When I assessed patient’s spiritual Domain, his son verbalized that before hospitalization he was very punctual in his rituals, offered prayers. Due to hospitalization he was unable to perform his rituals. During care when I asked about patient’s memorable day he stated that my memorable day is when me and my wife went to America to meet my daughter .we enjoyed a lot, visited the entire beautiful place and spend good time with my family. Suddenly patient stop conversation and cried. I shocked for a while, elaborating further he mention that he lost his wife. I felt very sad. The moment is very emotional, and my eyes are full of tears. I felt like crying. I felt the pain he was going through from which he can get relief after peace full death. I was very sorry for the absence of his family members, during his last movement who can give at least moral support for him. As a nursing student my responsibility is to provide holistic care to the patient and his family. During care I identified several affected domains, like physical (pain in leg and stomach, drowsiness), psycho-social (death of wife), spiritual (religious), and many others. Here all domains of palliative care are interlinked to each other but I focused on two affected domains: spiritual and sexual. This paper is about issues, intervention, recommendations regarding spirituality and sexuality. I would like to discuss the highlighted domain which was spirituality. Many patients find changes in their spirituality when they are at end of life. Some patients get very closer to God, offer their rituals and find religion as a coping mechanism to enhance their well-being. While some people get confused and stack in spiritual conflicts, blaming God for illness and denied from happening as result they separate themselves from supreme-power. Similar is the case with my patient, because of his illness his religious practice was greatly altered. As patient was CBR and has pain in his leg and right hand. He used to offer regular rituals prior, to his illness and now he is upset as he is unable to do. But he is very satisfied with his God’s blessing. He says that â€Å"God gave me everything, fulfill my wishes, and I am very happy with my life. But now he considers himself unkempt bec ause of urine bag, NG tube. I encouraged patient and his family to reconnect with God. Sulmasy, D. P. (2006). Stated that â€Å"spirituality play a vital role in a person’s coping style. It can also provide a network of social support that promote and to maintain emotional and psychological wellbeing†. As a nurse when I assess patients need, he verbalized that he wants to offer prayers. At that time I provide table covered with clean white clothes, so that patient put his tasbii on it and offer his prayer. Due to his physical illness he does not able to stand so, I encourage patient to offer ritual on his bed and I assist him with physical limitation. I curtain patients bed provided privacy and encouraged patient’s family to recite holy Quran near bed side, and assist to show the direction of kiblah. During feeding his son requested me to give holy water I give. The family really appreciates me and my effect comes fruitful. It makes me so happy and proud. Other highlighted domain was sexuality. Sexuality is fundamental desire for all human being. According to WHO as cited in (Tierney,D.K. (2008). â€Å"Sexuality is the combination of physical, emotional, intellectual, and social aspects of sexual beings in positive ways to enrich and enhance personality, communication, and love†. Most of the health care professionals are hesitant in asking about these issues. Health care provider always address physical, social, psychological domain and many times they neglect the domain of sexuality in their care. As a palliative care nurse it’s my responsibility to provide holistic care to patients so it’s important to discuss the impact of their illness on sexuality. Sexuality is not only about intercourse or not refer to just a physical act but it means identity, gender roles and orientations, libido, pleasure, and attachment. It is experienced and may be expressed through thoughts and feelings and love. It doesn’t matt er patient is so aged, weak or drowsy, but we have to assess patient sexual component. According to Julie (2004) sexuality is a lifelong natural need that is not limited by age, physical appearance, health status, or functional abilities. Referring to my patient’s sexual domain. No doubt patient’s wife is no more, he feel alone and misses his wife, he seems weak or aged but we have to assess patient’s sexual need, his feeling toward it. May be patient wants to meet his daughter share his feeling and emotion with her, gave any advice to them. As a nurse it is not necessary that I help patient to meet with his wife to fulfill his sexual desire, it doesn’t mean that. Delivering physical care also come in sexuality. Moreover, during our palliative course theory session, one of our faculties told us that physical appearance also comes under component of sexuality. If a person is looking well, his self-esteem also improves, while if a person is unkempt, his sel f-esteem declines which ultimately affects patient’s sexuality too. I would like to share a model that would further enhance our capabilities to understand the qualities of nurses that would help the nurses to play an effective role in palliative care. Gravity: Meaning of Life and Crystal Bowl Essay

Symbolism is a good tool to use when we come across difficult abstract concepts such as life and death. For example, in her article entitled â€Å"Gravity†, David Leavitt tells us a story about how a mom named Sylvia encourages her HIV infected son Theo to live longer. When Theo started to have bad eye sight in his childhood Sylvia just let him wear her flamboyant glasses despite that people around them all think that Theo looks strange. After Theo grew up, between a drug that could sustain his life and a drug that could save his sight, Theo chose the second one. Sylvia worried that Theo would give up the hope for living, so she tried to encourage Theo to live by a strange way which similar to the way she help him with his sight when he was a child. She threw a very heavy crystal bowl to Theo when they were shopping in a gift store. Theo caught the bowl without thinking. Theo realized that by throwing the bowl, on one side, Sylvia tended to prove that he is still alive. On the other side, Sylvia wanted to encourage him to live longer. The crystal bowl in the story represents exactly what Theo was going through. It symbolizes both life and death. It represents life because it was heavy and valuable â€Å"(The crystal bowl is) Four hundred and twenty- five dollars† (l. 88) This shows that the crystal bowl is very valuable just like that human’s life is very precious. In the same time, crystal bowl represent death because it could easily been shattered. Theo was surprised that he actually did not shatter the bowl â€Å"Theo looked at the floor, still surprised not to see shards of glass around his feet. † (l. 96-97) If the crystal bowl were broken, it would have not value or meaning just like it has lost its life. So, the Crystal bowl in the story represents death. If the bowl shattered, it just like a person lost his/her life. The crystal bowl also represents the character and hopes of Sylvia. The crystal in the story is very showing and pricy. â€Å"(The crystal bowl is) a large ridges crystal bowl, a very fifties sort of bowl, stalwart and square-jawed. † (l. 84-86) It represents Sylvia’s flamboyant and materialism characters. The author tells us that Sylvia has a flamboyant character by describing Sylvia’s bold and flashy glasses†Ã¢â‚¬ ¦ harlequins with tiny rhinestones in the corners†¦Ã¢â‚¬ (l.9) The author also implies that Sylvia is quite materialism by showing us that she care a lot about the price of a graduation gift her sister given to her son â€Å"Don’t you remember that cheap little nothing Bibi gave you for your graduation? It was disgusting. †(l. 50-51) On the other hand, the crystal bowl represents the strength of Sylvia’s hope for Theo to live because the crystal bowl is heavy â€Å"’That’s heavy,’ Sylvia said, observing with satisfaction how the bowl had weighted Theo’s arms down†(l. 93-94) The heavier the stronger, so the heaviness of the crystal bowl represent the strong will and hope of Sylvia. The author shows us that Sylvia is a strong willed woman who has done extremely stressful things in a role â€Å"The DHPG injections she (Sylvia) took in stride—she’s seen her own mother through her dying, after all, Four times a day, with the equanimity of a nurse, she cleaned out the plastic tube implanted in his chest, inserted a sterilized hypodermic a Sylvia and slowly dripped bag of sight-giving liquid into his vein†¦Ã¢â‚¬  (l.17-20) It shows that Sylvia has tremendous strength just like the toughness the crystal bowl appeared to be. The author also shows us that Sylvia’s hope for Theo to live is very strong. Even though Sylvia was stressed out †Yet she (Sylvia) had also, at about three o’clock one morning, woken him up to tell him she was going to the twenty-four- hour supermarket, and was there anything her wanted†(1. 122-124) This shows that Sylvia was on the edge of mental broken-down. But Sylvia still keeps strong hope for Theo to live â€Å"†¦ it had occurred to him (Theo) that she (Sylvia) was trusting his two feeble hands, out of the whole world, to keep it from shattering. † This shows that after all, Sylvia strongly hoped that, showing Theo that he was still strong by throwing the crystal bowl and letting him catch it, Theo would not continue on giving up his life but choose to live longer. The crystal also symbolized the general message which the author is trying to convey to the readers: life should be fully lived. The author shows that Theo was dying and tended to giving up his life. He felt â€Å"†¦wide and unswimmable the gulf was becoming between him and the evereceding shoreline of the will †¦Ã¢â‚¬  (l. 35-36) Even though he felt that way, he still caught the heavy crystal bowl without thinking. It shows the underlining deep and significant meaning of the story â€Å"There are certain things you’ve already done before you even think how to do them†¦ the bowl, which Theo was holding before he could even begin to calculate its brief trajectory†¦Ã¢â‚¬  (l. 131-133) This shows us that all human have an instinct for living. By showing us this, the story conveys a message to the readers that even though life is violable like the fragile crystal bowl; it doesn’t mean that we should give up. Instead, we should look at the positive side of life. Just like the crystal bowl also heavy and strong side, human lives also have a strong side because very one have instinct for living. Therefore, we should have faith to ourselves and stop complaining our own misery but start to look at life in a positive way, and realize that our lives should be fully lived.
